A Grants Pass man is dead and a Grants Pass woman is facing numerous charges for causing the fatal crash while she was impaired and then fleeing the scene.

The Oregon State Police said troopers and emergency responders were dispatched to what was initially thought to be a single-vehicle accident on Interstate 5 near milepost 59 on Saturday at 8:22 p.m..

Upon arrival, troopers found two occupants of a Volkswagen Jetta injured. They were identified as the driver -- 62-year-old Kim Douglas Parsley -- and his female passenger -- 18-year-old Ashlan Angel Parsley. Both were transported to Three Rivers Medical Center and Kim Parsley died at the hospital due to his injuries.

During the investigation into the cause of the crash, a second vehicle was identified and located by another state trooper about two miles from the crash scene. OSP identified the driver of the Subaru Legacy as 28-year-old Lindsey Danielle Johnson and she failed field sobriety tests after showing signs of impairment.

Lindsey Johnson was charged with 1st-Degree Manslaughter, 2nd-Degree Assault, Driving Under the Influence of Alcohol, Reckless Driving and Hit-and-Run Causing Injury. She was initially taken to the Sobering Center and then lodged in the Josephine County Jail.

OSP said the investigation into the fatal DUII crash is continuing.
